Please note that we do not accept insurance. If you would like to apply for reimbursement from your insurance company, you will be provided with a monthly invoice summarizing the services that you received and how much you paid for those services. It is recommend that you contact your insurance company before your first appointment and ask the following questions:

• Do I have mental health benefits?
• Do I have out-of-network benefits for outpatient therapy provided by an LCSW?
• Do I have a deductible that I have to meet before being eligible for reimbursement?
• How much will I be reimbursed for each appointment?
• Are there limits to how many sessions I can be reimbursed for in a calendar year?
• What steps do I need to take to apply for reimbursement?

Be advised that there is no guarantee that your insurance company will reimburse you, but we will make every effort to help you obtain reimbursement.

Before your first appointment, please print out the three forms below and complete them. Bring all three forms to your appointment. Any questions that you have about the forms will be answered at the appointment, but please call if you have any questions that you would like answered prior to your appointment.
